First Time Disability Services Student Information Form  (Required by all students requesting disability accommodation for the first time)

Semester Disability Services Accommodation Application (Fayette Campus students only - required at the beginning of each semester or term)

CGES - Semester/Term Disability Services Accommodation Application (CGES students - required one week before classes begin.)

Online Classes Semester/Term Disability Services Accommodation Application (All students taking online classes - required one week before classes begin.)

Alternative Text Agreement

Forms should be completed with the student's signature and sent to the Center for Learning and
Teaching by one of the following ways:

• Hand deliver to the Center for Learning and Teaching office which is located in Cupples Hall, room 320 on the Fayette campus.
