Your father's name as reported on your FAFSA does not match the name in the SSA records for his SSN. You must correct his SSN (Item 60) or his name (Items 60/62). If his name is correct, confirm by re-entering both his last name and first name initial in Items 61 and 62. If confirmed, your father should contact the SSA at 1-800-772-1213 or http://www.socialsecurity.gov and make sure they correct their records.
